New Patient Registration

Rinnes_viewPlease visit the health centres during normal working hours and tell the receptionist you would like to register.  You have the right to state a preference of doctor or nurse and to have this recorded in writing.

If you have your medical card, please bring it with you.  If you are registering your baby, please bring the form from the Registrar.

The receptionist will complete the registration form and make a screening appointment with the nurse or the doctor.
